Dental Crowns Straight From Our In-House Lab

The Forever Crown™ are created using digital scans so that the perfect fit can be achieved for every patient. Not only does this ensure comfort, but it also helps to ensure that the patient isn’t waiting for months on end while adjustments are made.

We will replace or repair lost, or damaged Forever Crowns provided the patient has kept routine professional oral hygiene appointments. If insurance benefits are available for replacement crowns, the insurance will be billed.

• Failure to keep the prescribed routine oral hygiene appointments (minimum every 6 months) will cause the warranty to become null and void.
• Damage or loss of Forever Crowns as a result of an accident that could break normal, healthy teeth will not be covered under the warranty.
• If the crowned tooth needs a root canal or there is recurrent decay on the tooth, this will void the warranty.
• In the event your account is sent to a collections agency for dental services rendered that have gone unpaid by the patient and/or the dental insurance, the warranty will become null and void for all Forever Crown purchases.
