A chipped tooth does not always require emergency care. A small chip confined to the enamel may cause little or no pain, though it still benefits from a dental assessment. Larger chips that expose the sensitive layer beneath the enamel, cause significant pain, or show visible pink or red tissue need prompt attention — and any chip that occurred alongside facial trauma, uncontrolled bleeding, or loss of consciousness requires emergency medical care first.

Chipped, Cracked, or Broken: What Is the Difference?

These terms describe related but distinct types of tooth damage. Understanding the difference helps explain why assessment matters — the surface appearance does not always reflect the full extent of the injury.

Chipped Tooth

  • A piece of enamel or surface tooth structure has broken away, leaving the remaining tooth largely intact
  • The chip is typically visible and may leave a sharp or irregular edge
  • Minor chips may cause little or no pain; larger chips can cause sensitivity
  • The root is generally not affected unless the chip is very deep

Cracked Tooth

  • A fracture line runs through the tooth — often not visible without magnification or diagnostic tools
  • Pain may come and go, especially when biting or releasing pressure
  • The crack may extend toward the root over time if left untreated
  • Diagnosis typically requires examination, bite tests, and X-rays

Broken Tooth

  • A larger section of tooth structure has broken away, often exposing the dentin or inner pulp
  • Typically more painful than a chip and more structurally significant
  • The fracture may extend below the gumline in severe cases
  • Treatment depends on how far the damage extends

Chip Severity: What Each Level Means

The depth of a chip determines how the tooth should be treated. An X-ray and clinical examination are the only reliable way to assess how far the damage extends.

Minor

Enamel Only

A small piece of the outer enamel layer has broken away. The dentin and the nerve beneath are not exposed. The tooth may have a sharp or rough edge.

May require: smoothing the edge, polishing, or composite bonding to restore contour.

Moderate

Into the Dentin

The chip has removed enough structure to expose the dentin — the sensitive layer beneath the enamel. Sensitivity to temperature and air is common.

May require: bonding, fragment reattachment if the piece was saved, or a crown if significant structure is missing.

Urgent

Nerve or Major Structure

The chip extends deep enough to expose or compromise the pulp, or so much structure is lost that straightforward restoration is not possible. Significant pain is common.

May require: root canal treatment before a crown, or extraction when the tooth cannot be restored.

What to Do Immediately After Chipping a Tooth

These steps help protect the tooth and manage discomfort until you are seen by a dentist.

    Rinse Gently

    Rinse with warm water to clean the area around the chipped tooth. Do not rinse forcefully.

    Save Any Fragment

    If a piece broke off, keep it moist in milk, saliva, or clean water and bring it to the appointment. The dentist may be able to reattach it.

    Avoid Chewing on the Affected Side

    Chewing on the chipped tooth can worsen the damage, especially if the edge is sharp or the remaining structure is fragile.

    Call the Dental Office

    Contact a dentist to describe the injury and arrange an assessment. Mention the size of the chip and whether you are in pain.

When to Call a Dentist and When to Go to the ER

Most chipped teeth are handled by a dentist. Some injuries that accompany a chip require hospital care first.

  • Significant pain or sensitivity at the chipped tooth
  • A sharp exposed edge that is cutting the tongue or cheek
  • Bleeding from the tooth or surrounding gum tissue
  • Visible pink or red tissue in or near the chip
  • Swelling around the tooth or jaw
  • The injury involved trauma, or the tooth feels loose

Go to the Emergency Room Now

  • Major facial trauma alongside the chip
  • Uncontrolled bleeding
  • Loss of consciousness
  • Difficulty breathing or swallowing

Call 911 or go directly to a hospital emergency room for any injury involving the airway, head, or significant facial trauma.

For most chipped teeth without accompanying trauma, a dental office is the appropriate first contact. Call ahead to describe the injury.

What Happens at the Emergency Appointment

The appointment begins with understanding the injury before any procedure is discussed.

    The dentist examines the chip, the surrounding gum tissue, and adjacent teeth. The extent of missing structure, any sharp edges, and signs of nerve exposure are assessed.

    An X-ray is taken to evaluate the root, surrounding bone, and whether the pulp is affected. Imaging reveals damage that is not visible at the surface.

When significant structure is missing but the root is intact, a crown restores the full shape and protects the remaining tooth.

Root Canal Treatment and Crown

When the chip has reached or compromised the pulp, root canal treatment addresses the nerve before a crown is placed to restore the tooth.

Extraction

When the chip is severe enough that insufficient tooth structure remains to support a restoration, extraction may be the most appropriate option. Replacement options can be discussed after healing.
