Please keep in mind that dental insurance contracts are subject to change. For this reason, check with your insurer to make sure Silver State Smiles and Dr. Duboff participate as in-network providers for your dental plan. If we’re in-network for your plan, we’ll be able to submit your claim. If not, you may be responsible for paying your bill directly and then submitting your claim to your carrier for reimbursement.
Most insurance companies offer a variety of benefit plans with different features. Co-workers or friends who are covered by the same plan as you may actually have coverage that differs from yours. Please be sure to check with your insurer to understand your plan’s specific coverage. Our best advice is to review the benefits information about your specific plan before visiting our office.

Please note these important points about your coverage.
- If you’re entitled to benefits from more than one group dental plan, the amounts paid by the combined plans will not exceed 100 percent of your dental expenses. Benefits for dependents vary from plan to plan. Pay particular attention to special clauses and to language about dependents.
- Dental benefits are calculated within a “benefit period”, which is typically for one year but not always a calendar year. Check your benefits information so that you know when you might be approaching your deductible payments or plan maximums.
